Waiver of Liability, Assumption of Risk, and Indemnity Agreement

This is a mandatory form in order to participate in any or all events, activities, or trips in the Leadership and Service Living Learning Community at UC Merced. You must complete the Waiver of Liability, Assumption of Risk, and Indemnity Agreement before you participate in any outdoor, in person, and/or service activity. 

If 17 or younger, guardian permission will be required due to off-campus participation.
